The infection required debridement of discontinuous areas on three extremities as well as on the trunk. Klebsiella sp necrotizing fasciitis is a rare entity and has not been described previously in concert with heart transplantation. Klebsiella cases classically have skip lesions as described herein. Klebsiella oxytoca is a rare cause of necrotizing fasciitis in North America and presents with multifocal infection sites. This along with the patient's blunted immune response after organ transplantation necessitated a high index of suspicion for diagnosis and treatment. PMID: 18971102 [PubMed - in process]

Recognition of this novel PTLD pattern as a potential imitator of acute rejection in particular is important because reduction of immunosuppression (to treat PTLD) is antithetical to the usual therapy for rejection. J Heart Lung Transplant. 2008 Nov;27(11):1251-6 Authors: Kallistratos MS, Dritsas A, Laoutaris ID, Cokkinos DV BACKGROUND: N-terminal pro-brain natriuretic peptide (NT-proBNP) plasma levels have been associated with indices of left ventricular (LV) function and aerobic capacity in heart failure. METHODS: We prospectively followed-up 149 patients with impaired left ventricular function for 30 +/- 10 months. During this period, 22 patients died and 5 underwent heart transplantation. Blood samples for NT-proBNP assessment were taken at baseline and before cardiopulmonary exercise to estimate peak oxygen consumption (Vo(2)). LV cavity diameter, left atrial size and LV ejection fraction (LVEF) were measured by echocardiography. RESULTS: NT-proBNP plasma levels >1,164 pg/ml showed 85% sensitivity and 82% specificity for detecting Vo(2)<14 ml/kg/min (area under the curve [AUC] = 90%, p < 0.001). Patients above this cutoff showed a 13.6-fold greater hazard ratio compared with those with values below this cutoff (p < 0.001). NT-proBNP plasma levels of >760 pg/ml showed 77% sensitivity and 69% specificity for detecting LVEF <28% (AUC = 77%, p < 0.001). Patients with values above this cutoff showed a 15.85-fold greater hazard ratio compared to those with values below this cutoff (p < 0.001). The addition of NT-proBNP to an assessment model that includes peak Vo(2), LVEF and New York Heart Association (NYHA) classification can significantly improve predictive ability. CONCLUSIONS: Assessment of NT-proBNP should be performed to detect candidates for heart transplantation because of the useful prognostic information that it can provide. Determinants of bioartificial myocardial graft survival and engraftment in vivo.

The aim of this study was to describe and quantify the fate of a tissue-engineered cardiac graft in vivo. METHODS: Neonatal rat cardiomyocytes were cast into a collagen mesh, forming a bioartificial myocardial tissue (AMT). After 7 days in vitro, four groups were formed (Group A: sham; Group B: matrix; Group C: AMT [with additional host treatment with cyclosporine and prednisolone]; Group D: AMT; each n = 5) and the tissue grafts were implanted into the muscle pouch of adult rats at 14, 21 and 28 days. Implants were stained for troponin-T, BrdU, MF-20, desmin, vimentin, Flk-1, CD8, CD4, pentachrome, PSR and H&E. RESULTS: AMT cell count, cell proportion, contractility, viability and metabolism proved stable in vitro. Grafted cells decreased over time and were detected in Group C until the end of the experiment (Day 28), and in Group D until Day 21. Angiogenesis began at the peripheries and slowly progressed toward the cores of the grafts. The thickness and collagen content of the matrix remained stable in Group C for 14 days, and decreased in all groups until Day 28 (thickness: Group B, -66%; Group C, -50%; Group D, -100%). Grafts were predominately infiltrated by macrophages and stromal cells, and less so by lymphocytes (Group D > B > C). CONCLUSION: The differentiation of cardiac and non-cardiac grafted cells, infiltrating cells, scaffold kinetics and angiogenesis showed host immune responses and degree of angiogenesis to be the determinants for AMT graft survival.
